Abstract:
A patient self-report survey to assess health plan or provider performance for chronic disease. Includes questions on receipt of evidence-based care, symptom and severity level, self-care management, physical and emotional functioning, and physician interaction for people with asthma. Designed to be used with the FACCTONE CAD survey and FACCTONE diabetes survey. The FACCT|ONE surveys assess essential health system competencies common to many chronic illnesses and not generally addressed in other widely used accountability measures.
